I noticed that the cleaning people closed all of our bathroom doors, which I found very annoying. I had to knock on them before entering - and I have this thing about not wanting to disturb someone in the bathroom, so it made me very unhappy to be put in that position.
That must be the default at their homes. What do you do?

Leave all doors open. It tells lets others know it is unoccupied, and sets the standard for leaving a space in good order. If it is too messy to leave the door open, your task is not finished.
On a side note--our family rule has always been a closed door for privacy, but NEVER locked.

They are probable not U.S. citizens (or are naturalized). In America, unoccupied bathrooms often are left open, only when the bathroom is occupied is the door closed. Everyone knows this (here in the U.S.) so often bathroom doors are left unlocked when occupied. We just wait until the door is open to go in. In other counties the door is always closed as it's not cooth to ever see into the bathroom. So, they always lock the door when occupied. Can you see the potential embarrassing situations here? The problem this creates is that if I am in the bathroom, not only will I have to lock the door (I never do at home), but in their world they will come and turn the handle before discovering it is locked, and then I have to yell "I am in here." Yes. Guests at the house ranging from my kid's friends to my parents leave the cover up and it really upsets me enough that I am considering making a sign. If my dog's water bowl is empty, I don't want him drinking from the toilet. Also I don't want a small child drowning in it - yes that happens too often.
Yes. Cleaning people put it on backwards and drives me nuts.

Doors open here. It lets you know it's unoccupied and lets air circulate.
If this discussion was on a socialist website the solution would be a law requiring doors be left open a requirement for elaborate bathroom doors that can recognize occupancy. It's for the children.

Bathroom door open is bad feng shui. Not that I believe in it, but anything that focuses your attention on your surroundings is a good thing, be it rituals or routines.

I hybrid. The door is technically open when unoccupied, but pulled almost to close. The darkness of the bathroom is the confirmation that it is unoccupied. I prefer this arrangement particularly at night so that I only have to push the door, and do not have to make any noise on the handle or latching mechanism.

I think cleaning people close doors because they've been trained that way or to keep track of what rooms they've cleaned.

Toilet seats and lids should be down for reasons of common courtesy (to the ladies especially) and overall proper etiquette. Seats down also serves well for aesthetics... as someone entering in to the bathroom for other reasons has no need to see inside of the toilet.
