One minute and 39 seconds of pure awesome. Notice that the guard limp wrists his Glock and instantly reduces the malfunction. Excellent training leads to good results. Too bad he didn't use two hands.
Also notice how he never crouches or manuvers, and that he lost ability to smoothly interact with his environment when trying to open the door. These are all very natural things under combat stress.

There's a special name for a gun that doesn't run when "limp wristed": Broken.
Simple. Get it fixed so it always runs. This is an excellent example how you might not have the perfect grip or position in a defensive situation. If your defensive gun doesn't run with a less than an optimal grip, it needs to be worked on so it does, period.
For my defensive guns after running several hundred rounds through them to reach a good level of grime, my final test is fire them as limp wristed as possible (weak hand only, elbow bent, wrist relaxed, about a foot from my face -- they just about hit me). They all not only run, but lock back after the last round.
Don't blame the shooter ("limp wristing"). It's a gun problem.
