Form 1 on a Shockwave

Thinking about stamping my Mossberg Shockwave to a SBS.

On the form 1 under model should I just put 590, or 590 Shockwave?

Doesn't have Shockwave on the weapon, only place it has it is on the box.

Wait time is around 8 to 12 months still?

I would use what is stamped on the receiver.

I've read that eforms are coming back as fast as 10-12 days!

Use the model number that is stamped on the weapon.

Further, e-filing is taking upwards of a couple of weeks to approval now. If you can get the darn thing to work.
