I've never filled one of these out before. All my items are purchased and owned by my trust, is it necessary to fill this out? My dealer sent one with the forms to me and said some have been getting kicked back if they didn't have this filled out.

Your dealer is correct. It's a crap shoot if it'll get kicked back or not. It's simple enough to fill it out and send it, just to be sure.

May depend on your examiner. I believe our TX gal (Ms Jones) has said it's not necessary for her on a trust.

I have customers in TX that have been kicked back for not submitting.Wahnsinn wrote:May depend on your examiner. I believe our TX gal (Ms Jones) has said it's not necessary for her on a trust.
I think it has more to do with who the supervisor on duty is or how old the coffee is.

So under question 1 do I put my trust name or my name?Bendersquint wrote:I have customers in TX that have been kicked back for not submitting.Wahnsinn wrote:May depend on your examiner. I believe our TX gal (Ms Jones) has said it's not necessary for her on a trust.

If I put my trust name in #1 do I answer #2 with my actual name?

Nope it is the standard form required for all individual transfers and entities, but examiners determine when they want them from entities based on a magic 8 ball i think.ericlw wrote:im in texas I just got my suppressor 2 days ago.I also received one over a year ago. Might as well provide the form, even if you think it won't be required. Given the hassle if they return your paperwork, it's not like it's a major effort to fill out.
BTW there's an updated version of this form, relative to what is shown above. Download from atf.gov.

Worse case scenario is they say its not needed and it goes in a burn bag.
Or you can have a several week delay because it was needed and it wasn't included.
Smart way is to just include it. 5 minutes to print and fill out sure beats an additional 2 weeks delay in my book.

I got a Form 1 returned last week for no Certificate of Compliance. Bummer! Sent one in the day after I received the letter. Sent another for another 2 Form !'s ~30 days behind the first...

I always send one in with my Corp transfers, if they don't need it then they can shred it.

s--t!! I forgot to submit one with my last two forms.
I filled out two sets of form 4's, two $200 checks and dropped off two copies of my trust Memorandum, Declaration, Ammendments with both updated and former Schedule A (depending on which the examiner wanted to see). I dropped them off at my dealer on the way to the airport so that when the cans arrive he can just turn around and mail everything in for me since I'll be out of town.
Reading this thread I realize forgot the certificates of compliance...
Hopefully the SDN6 hasn't arrrived yet and the packet hasn't been sent in.

My first 4 stamps went through without one on a trust and no issues. I now include it though in every new packet I send. It's stupid easy and doesn't require a notary so why not?
