Gemtech Address on Form 4

Trying to fill out a Form 4 for to transfer between private parties. The Form 4 the Gemtech silencer is on now was e-filed and missing address in Box 4a.

I figured it's either:

GEMTECH
P.O. Box 140618
Boise, ID 83714-0618

or

GEMTECH
335 North Edgewood Lane, Suite #150
Eagle, ID 83616

Can someone let me know which they have on their Form 4?

Use the second address in Eagle, ID. ATF wants physical addresses, no PO boxes anywhere.

What did the previous Form4 have on it?

Consistency of records means no error letters.

Previous Form 4 from dealer to individual had no address as it was e-filed. I feel like there should be an address regardless.
