Has anyone used the new forms yet?

Can you send 2 forms in (for 2 items) with one responsible person questionnaire or do you need two of those?

Also how many fingerprint form are required??? Do have to sent 2 or 4 cards for 2 NFA items?

Each form submitted requires its own set of paperwork and fingerprint cards.
