Call Joe @ Curtis Tactical. I have a form 1 that didn't turn out the way I had hoped and I sent my less than stellar product to him and he transformed it into more than I expected. The form 1 you have as certain "requirements" that must be met as in length, MDL #, SER # primarily so finding a suppressor from an SWR, Gemtech etc is not gonna happen. It will need to be produced to match the paperwork you currently have and what the stamp is actually for.
