Form 1, or 2 or 4 and handgun fore-grip

I would like to add or attach a foregrip to a handgun that was a rail system around the barrel.
I see where a NFA manuafacturer can do it and transfer it to me for ATF cost of $5.00, but how much does it cost that manufacturer to submit the form?

Or do it myself on a form 4 for $200.00.

If you do it you're self it would be a form 1 for 200 bucks.
