Form 4 question

So I just got back my latest form 4 for an omega suppressor. All of my previous form 4's came back with a stamped "signature" from the examiner as well as the date filled in. On the form I got today after picking up my can I noticed these fields were empty and neither the approved nor disapproved boxes were checked.

Does the ATF no longer fill in these fields on the copy they send back or do I have a fluke form that needs to be corrected? The stamp is there as per normal with serial number and an initial (looks like a capital B but can't be sure).

What do you guys think? This is only my fifth NFA item and don't know if they changed procedure in the last year or so.

OK so just to be clear no one has ever received their form 4 back with these fields blank?

Well if anyone else runs across this I called the ATF and they asked me to send the form back in so it could be signed. Hopefully the turnaround doesn't take too long.
